Tzu Chi Melaka is collaborating with Pantai Hospital Ayer Keroh (PHAK) to provide financial assistance for free Cataract Surgery to the underprivileged. Tzu Chi Melaka will assist in getting the eligible candidate, and PHAK will do the surgery. For those who are eligible and interested, kindly click the link to apply. 1. For Malaysian citizen & permanent resident of Malaysia. Permanent resident must either be born or lived in Malaysia since pre-independence or have resided in Malaysia for minimum 30 years. 开放给大马公民及永久居民申请。永久居民必须在独立前出生或居住于大马至少30年。

2. Gross individual salary not exceeding RM5000 per month, including basic pay, allowances, OT, commission and any perks which form part of applicant’s earnings, fixed or otherwise. 个人收入每月不超过RM5000， 包括底薪/佣金/超时津贴等任何属于申请者的个人收入。

3. Gross household income not exceeding RM5000 per month (included family members), including contribution from family members not living with applicant and any company/organization towards the applicant. It is inclusive of basic pay, allowances, overtime, commission and any perks which form part of the overall earnings, fixed or otherwise. 家庭收入(包括子女)不超过RM5000，包括沒与申请者住在一起的家庭成员 (若相关家庭成员有提供任何补助)，或任何机构的捐款。也包括了基本工资、津贴、加班费、佣金和构成总收入一部分的任何津贴，无论是固定还是其他

4. Recommended by resident consultant at Pantai Hospital Ayer Keroh.  必须由爱极乐班台医院专科医生推荐。

5. Each beneficiary can only benefit from 1 procedure from Life Renewed. Those who has benefited from previous funding of Khazanah is not considered. 每人只能享有一次援助机会。若之前已享有Khazanah援助治疗的计划，不能再次申请。

6. Beneficiary should not be covered by any form of insurance or employment medical benefit. 申请者沒有购买任何保险保单或享有雇主医药福利，方可申请。

Compulsory Documentation 所需文件: 

1. Photocopy of MyKad/MyKid/MyPR. For MyPR holder, a valid or certified document verifying the holder’s total length of stay in Malaysia must be provided. 申请者身份证/MyKid/永久居留证。永久居留者必须提供文件证明已在大马居住的年限。

2. Latest 3 months’ pay slip (for the employed within the same household). In absence of salary slip, a letter of confirmation from the employer or registered Malaysian NGO must be provided, detailing the total monthly income received. 申请者最近3个月的薪水单 (也包括同一家庭内的雇员)。若沒有薪水单，必须提供雇主的证明信或NGO的详细报告说明有关收入与支出。

3. Latest bank statement/EPF statement/income tax statement (for unemployed within the same household). SOCSO statement must be provided for applicants who are unemployed or disabled/handicapped. 申请者最新的银行户口结单/EPF结单/所得税结单 (也包括同一家庭内成员)。若残障人士沒工作，需呈交SOCSO 文件证明。

4. Photo of place of residence, front house, backyard, kitchen, living room and bed-room of residence, at least 5 pieces. 申请者的住家照片，屋前及屋后，客厅，厨房及睡房，至少 5 张照片。
