Winter Sports Tryout Information Form
ALL items need to be completed before a student can come to Tryouts on Nov. 15th: 
1) Complete this Form so that our Staff can check & clear students Academically...
   a student's eligibility is based on grades & attendance from 2nd Semester of Last Year
   although student's who make a team will need to maintain eligibility to stay on the team.
2) Register/Complete all Information to DFM; http://dragonflymax.com/forms/LJWB3P
   upload a current Physical Exam & Medical Eligibility signed by Dr's Office to DFM
*) Boys will also need to return a signed copy of Coach Rouse's Team Rules by Nov. 15th
Make sure you have ALL Forms Completed, Signed, and Uploaded to Dragonfly Max 
OR Turned into Coach Medinas in room 6210 so you can be cleared by Athletics Dept.
Do all of this Before tryouts begin (Nov. 15th - 17th) or you may not be able to tryout
(Can be turned in at Tryouts but if anything is missing you won't be able to tryout that day)  
Meet in the Ligon Gym after school for your Tryouts with your Gym Clothes & Sneakers.

A First or Final Cut will be made on Friday, Nov. 17th
Anyone making the 'Cut' on Fri, Nov. 17th will come back the following week...  If another cut is needed it will be made on Tuesday, Nov. 21st before the Break...
